2024 Conference Tribute Ad Order Form

Thank you for making the decision to honor your Executive Director or Temple Administrator with a Tribute. You may use this form to submit your ad. Once completed, we will send you an invoice for payment. Payment may be made online with a credit card, debit card, or ACH bank transfer. The ad will not be displayed if payment is not received before the December 1st deadline.

About the Honoree

Select which Tribute option you want to purchase. Note that the Video Tribute option INCLUDES a printed & mounted keepsake Tribute as well. You will need to upload the printed ad file and send an email to us with the video tribute file.

  • 11" x 8.5" (standard letter size in landscape orientation - this means horizontal orientation, not vertical)
  • PDF, jpeg, or png format
