Ethics / Code of Conduct Violation Report

This form is for reporting of experiences of violations of the NATA Code of Ethics or Code of Conduct. Your submission will be viewed only by the NATA Executive Director and members of the NATA Ethics Committee.

We ask you provide your name and contact information so we may follow up with you about your concern. If you do not feel comfortable submitting a report in writing, you may reach out to the NATA Executive Director, Alison Siegel Lewin, at 800-966-6282 to discuss.

Please note: If you fear for your own welfare during the NATA Conference or other NATA event, and especially in the case of immediate concern or danger, please contact NATA staff on-site at the event or venue security, or please dial 911.
